Valentine's Day Makeover with Matt & Meribeth Contest Details:
Contest Dates: Friday, Jan. 20 through Thurs., Feb. 2nd, 2012
E-mail or mail Take Five a letter in 50 words or less, telling us why you or someone you know deserves a makeover. You must include a current photo, and your entry must reach Take Five by 10am ET on Thursday, Feb., 2, 2012.
Take Five will select one winner 18 years old or older to receive an image consultation and new outfit from Younkers Department Store of Grandville (Rivertown Crossings Mall), hair and make-up services from Matt Flora Hair Studio, and nail service from Chic Nail Spa of Grand Rapids!
The winner will be announced live on Take Five on or about Monday, February 6th, 2012.
The winner must be available on Friday, Feb. 10th, 2012, from 9am to 3pmET to receive the makeover services and agree to appear in video for broadcast on WZZM 13/Take Five.
The winners must agree to cut and color their hair, and undergo minor facial hair removal.
The winner's new look will be revealed live on Take Five Monday, February 13, 2012, however WZZM 13/Take Five reserves the right to make adjustments to the reveal schedule and live and/or taped appearance of winner.
The winner must be available and agree to appear live on Take Five for the scheduled reveal on Monday, February 13, 2012, from 8:00am-10amET

The "Make Me Over with Matt & Meribeth Contest" is open to residents of the state of Michigan. Entrants must be at least 18 years of age or older. Employees of WZZM 13, Gannett Co., Inc., Younkers Department Store, Matt Flora Hair Studio, Chic Nail Spa and each of their respective affiliated companies, subsidiaries, advertising or promotional agencies, and other agents, and the immediate family members of, and any persons domiciled with such employees, are not eligible. The Contest will begin on Thursday, February 2, 2012, and all entries must be received by 10am ET on Thursday, February 2, 2012.
Entries may be submitted by U.S. Mail (U.S. Postal Service) or by email. Entrants must send a letter (50 words or less) and attach a photo, telling Take Five why they deserve a makeover from Younkers Department Store, Matt Flora Hair Studio and Chic Nail Spa. All entries must be received no later than 10am ET on Thursday, February 2, 2012. Only one entry per envelope or email. Take Five producers or Professional Image Consultant, Meribeth VerHulst of Younkers Department Store or Matt Flora of Matt Flora Hair Studio will contact the winner on or about Friday, February 3, 2012. The winners will receive a personal new look makeover by Younkers Department Store of Rivertown Crossings Mall, Matt Flora Hair Studio, and nail services from Chic Nail Spa of Grand Rapids.
The value of the Younkers Department Store prize is $250.00. The value of the Matt Flora Hair Studio makeover package is $125.00. The services include color, cut & style, facial waxing and makeup. The value of the nails service from Chic Nail Spa is $50. The approximate retail value of the total prize package is $425.00.
By participating, entrants agree to be bound by the complete Official Rules and the decisions of the judges. The winners must be available and agree to appear live on Take Five for the scheduled reveal on Monday, February 13, 2012, from 8:00am-10amET. WZZM 13 and Take Five reserve the right to make adjustments to the reveal schedule and live and/or taped appearance of winner. Winner is responsible for all applicable taxes.
If required by law, you will be sent a 1099 form for submission to the IRS at the end of the year.
