GRAND RAPIDS, (WZZM) -- Leaving the hospital sounds simple, but many times patients find themselves back in the hospital within weeks of going home.
A national report released by the Robert Wood Johnson Foundation examines hospital readmissions. People that leave the hospital healthy are returning just a short time later due to the condition that brought them there in the first place. Without high-quality care coordination, patients bounce from home to the emergency room and back to the hospital.
Almost one in five elderly patients released from a hospital is back within 30 days and more than one in three are back within 90 days.
