Senior Wellness: PACE Programs helping seniors

One of the best kept secrets in healthcare for seniors is PACE, which stands for "Program of All-Inclusive care for the Elderly."

One of the best kept secrets in healthcare for seniors is PACE, which stands for “Program of All-Inclusive care for the Elderly."

PACE programs offer long term programs, and access to care 24 hours a day for older adults with complex medical or memory needs.

PACE has existed across the U.S. for 40 years, and there are 13 PACE sites in Michigan. Most provide the same basic services, by coordinating and providing medical and social care for seniors. The programs work with adults to define what they want, and make sure their care helps them achieve what matters most to them.

In West Michigan, there are several locations that can help.

CentraCare –Two Locations serve Calhoun County, Kalamazoo County, a portion of VanBuren County
Care Resources – One location serves Kent County and a portion of Ottawa County
LifeCircles PACE – Two locations serve Muskegon County, Most of Ottawa County, and a portion of Allegan County. Porter Hills is a partner of this PACE program.
